The only TEFL internships that I'm aware of are run by TEFL Course providers who see it as a way to lure trainees into buying their courses.

One way of getting online work is through some of the online agencies (forget their names but you can google them) BUT you won't get anything like your target rates through them. Best way for those rates would be to find your own "clients" (clients should pay better than students, right?) ...

I would add that as you get more experienced, preparation time goes down. In fact, at the very beginning it could actually be more than 20%. Preparation is one of the most important things. It's what gives you confidence in the (online) classroom.
